WAIVER OF LIABILITY: In consideration of participating in Alpha Female League [its subsidiary and or affiliates] programs, activities, events, workshops, sports, classes, etc. I acknowledge that I have read and understand the nature of these activities and that I/my minor child is qualified, in good health, and in proper physical condition to participate in such activities. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity or will immediately pull my minor child from participation. I fully understand that these activities involve risk of serious bodily injury which may be caused by my own actions, or inactions, those of others participating in this event, the conditions in which the event takes place, or the negligence of the "releases" named below; and that there may be other risks either not known to me or not readily foreseeable at this time: and I fully accept and assume all such risks and all responsibility for losses, costs, and damages I/my minor child incur as a result of participating in the activity. I understand that the rules set by Alpha Female League and or any other activity associated with the organization, I/my minor child may take part in, must be followed at all times while participating or being in, on or around the premises of said event(s) and failure to abide by the rules may result in injuries or penalties at my own expense.

I hereby release, discharge, and covenant not to sue Alpha Female League, its subsidiaries, affiliates, respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and where applicable, owners and lessors or premises on which the activity takes place (each considered one of the "RELEASES" herein) from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence or the "releases" or otherwise, including negligent rescue operations; and I further agree that if, despite this release, waiver or liability, and assumption of risk I, or anyone on my behalf or behalf of my minor child, makes a claim against any of the RESEASES, I will indemnify, save, and hold harmless each of the RELEASES from any loss, liability, damage, or cost which may incur as the result of such claim.

PHOTO & MEDIA RELEASE: I or as the parent/guardian of the named participant, hereby consent to the use of, reproduction of, editing and/or broadcast of any and all photos, audio, videos and or content created by Alpha Female League or its subsidiaries, affiliates, partners, etc.  with me and/or my minor child[ren] taken by or on behalf of Alpha Female League without compensation to me or them. All negatives and positives, digital or printed images, audio/video-recording, etc. shall constitute the property of Alpha Female League solely and completely. I am not and will not lay any current or future claims to such audio, images or recordings. Alpha Female League may use those mentioned for purposes they deemed so for advertising, marketing, social media, etc. without any compensation to me or my minor child, now or in the future.

PARENTAL CONSENT: I or the parent/guardian of the named minor understand the nature of the above referenced release and waivers. I AGREE TO THE INDEMNITY AND HOLD HARMLESS Alpha Female League [AFL] and its affiliates, etc. from all liability, claims, demands, losses, or damages on behalf of myself and my minor child, caused or alleged to have been caused in whole or in part by the negligence of AFL or otherwise, including negligent rescue operations, and further agree that, I, and or on the behalf of my minor participant, or anyone else on behalf of myself or behalf of my minor child to make any claims against AFL, is subsidiaries, affiliates, volunteers, staff, interns, sponsors, partners, etc. I WILL INDEMNITY AND HOLD HARMLESS each of the forementioned from any litigation expenses, attorney fees, loss liability, damage, or cost that may occur or incur as the result of any such claim.

By initialing and signing this form, you acknowledge and agree that you have read this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT. You also acknowledge that by signing, you understand that you are giving up any substantial rights and that you are signing signed freely and without any inducement on behalf of yourself or your minor child[ren].
